PTs: Do No Harm but What About Diet?

The Oath

As many PTs and healthcare providers know and affirm: first, do no harm. Taken from one of the most widely known Greek Medical Texts, this oath commits doctors and healthcare providers to offering their best in care. The more modern version of this oath expounds:

"I swear to fulfill, to the best of my ability and judgment, this covenant: I will respect the hard-won scientific gains of those physicians in whose steps I walk, and gladly share such knowledge as is mine with those who are to follow."

Many physical therapists (PTs) have taken a similar oath. In DPT schools around the country, many student PTs (SPT) swear an oath to provide their best care prior to going into internship or clinicals. Upon entering the profession and joining the American Physical Therapy Association (APTA), we as PTs swear by the APTA code of ethics, which outlines: "Above all, do no harm." The APTA code of ethics also asks PTs to provide our best ability and judgement, among other requirements.

The Most Important Nutrient for Arthritis

There's always a blog or hot article on your newsfeed talking about a "miracle" nutrient or special vitamin. While many of these articles are complete clickbait or mere fluff to get you to buy their supplements, this article has some science and experience behind it. Now that we have that settled, I want to explore the one nutrient that I think has the biggest impact on clients with arthritis: fiber. 

Arthritis comes in several forms but the 2 most common types are osteoarthritis (OA) and rheumatoid arthritis (RA). We as physical therapists (PTs) often help clients with both of these diseases. From exercises, manual therapy, modalities, dry needling, and education -- our roles as PTs is to help our clients move better and reduce their pain. Nutrition and Chronic Pain

Many physical therapists (PTs) treat patients with chronic pain. Chronic pain can come in many forms. From autoimmune diseases like rheumatoid arthritis (RA) or fibromyalgia, to centralized pain phenomenons like allodynia and hyperalgesia, chronic pain can cause marked disability, loss of functioning, and lacking participation in life.

Integrative Pain Management

Physical therapy (PT) can offer significant benefits to many patients with chronic pain using traditional PT methods. Exercise, stretching, manual therapy, stress reduction, pain science, mindfulness, and activity modifications are all mainstay PT treatments for people with chronic pain. PTs are now starting to see the evidence for how nutrition can also help patients with chronic pain; and as more physios include nutrition in their practices, their outcomes are improving quite dramatically.

Arthritis, PT, and Weight Loss

Last week, I slammed the Journal of Orthopedic and Sports PT (JOSPT) for not including nutrition in their CPGs regarding concussions. This week, I want to laud them for including a great meta-analysis on knee, hip, and spine osteoarthritis (OA) in relation to weight loss in this month's journal.

As physical therapists (PTs), we know that musculoskeletal disorders like knee, hip, and back OA account for a huge portion of healthcare spending. We also know that OA of these joints greatly impacts quality of life, function, and ultimately long-term disability. Physical therapists do their best to offer cutting-edge treatment like exercise and patient education.

Current research shows that people with arthritis can greatly reduce their pain, improve their movement, and restore their strength with physical therapy. Did the Concussion CPG for PTs Go Far Enough?

Last month the Journal of Orthopedic and Sport Physical Therapy (JOSPT) published a lengthy Clinical Practice Guidelines (CPG) specific to physical therapy and concussion management. Here's a link to the authors' summary. The authors of the CPG offered a long list of references, well-thought out discussions, and clear evidenced-based recommendations. However, our team at Nutritional Physical Therapy feel the CPG did not go far enough for physical therapists (PTs). 

Managing patients with concussions, or mild traumatic brain injuries (TBIs), involves a multidisciplinary and multi-faceted approach. Concussions involve disruptions in cognition, planning, executive function, balance, physical performance, speech, vision, and even sleep. The European Society for Clinical Nutrition and Metabolism, also known as ESPEN, have released pre-published guidelines for clinicians to consider for patients with COVID-19 (novel Corona virus). The guidelines offer a great breakdown of evidenced-based statements related to managing patients in the intensive care unit (ICU), outpatient and those in quarantine.

Physical therapists (PTs) are considered essential during the COVID-19 crisis. Physical Therapists and Corona Virus: What to do?

Our world has changed drastically in the last month with the progression of the novel Corona virus, also known as COVID-19. A stock market crash, travel halting, to thousands of people dying, COVID-19 is taking its effect rapidly. Many business owners and workers across the world, physical therapists included, are being impacted by this virus. What are physical therapists to do with all of this time and lack of productivity? My response is to keep learning!

I was having a great chat with Erson Religioso (aka The Manual Therapist) last week. He too was frustrated with being at home. Sitting idle does not suit either Erson's or my personalities.
