Mechanical loading builds bone, but metabolic bioavailability dictates whether the skeletal matrix actually forms. While filming in Costa Rica’s Nicoya Peninsula, the biological driver behind the local centenarians' exceptionally low hip fracture rates became glaringly clear: deep-aquifer hard water.
Key Clinical Insights for Physical Therapists
Magnesium is the Neuromuscular Gatekeeper: Calcium triggers muscle contraction, but magnesium drives the active clearance of calcium back into the sarcoplasmic reticulum. Patients presenting with persistent hypertonicity, chronic muscle cramping, or delayed post-rehab recovery often suffer from mineral depletion rather than pure mechanical overload.
The Trap of Isolated Supplementation: Recommending high-dose synthetic calcium pills without balancing Vitamin D3, Vitamin K2, and magnesium increases the risk of vascular calcification rather than building cortical bone density.
Hydration Quality Over Volume: Fluid balance is fundamentally an electrolyte conversation. True cellular hydration requires mineral-rich fluids capable of supporting efficient action potential propagation across neuromuscular junctions.
Screening Protocol for Initial PT Evaluations
Water Source Audit: Identify if patients rely exclusively on heavily purified, reverse-osmosis, or distilled water without re-mineralization.
Supplement Review: Check for high-dose isolated calcium carbonate pills (>500 mg) missing essential cofactors like Vitamin K2 or magnesium.
Symptom Triad Correlation: Flag the common overlap of osteopenia, nocturnal calf cramps, and chronic joint stiffness as a potential indicator of electrolyte imbalance.

Hey y'all, Dr. Wells here coming from Nicoya Peninsula in Costa Rica, and today we're talking (0:06) about calcium. So bone mineral density is a huge topic, right? And a lot of people want to pop those (0:12) calcium supplements, but interestingly here in Costa Rica, particularly in this area of Costa (0:16) Rica, the water has high concentrations of minerals like calcium and phosphate and magnesium, and so (0:22) naturally what ends up happening is they drink the water, they get that natural source on top of (0:27) maybe from other sources like cow's milk and things, and their bones are some of the strongest (0:31) in the world. Rates of hip fractures here are very low, plus you compile that with the hiking, the (0:36) walking, the surfing, they have very low rates of spontaneous fractures and falls and fractures.
So (0:42) food for thought for those that are popping those pills and supplements, particularly calcium, (0:45) it interacts with lots of other supplements and medicines and drugs, and something we can learn (0:49) from here in Costa Rica. So moving forward in the future, consider get your minerals from your water, (0:55) get your vitamin D from the sunshine, and keep your bones strong. Cheers.
